Reapply for a Massage Therapist Permit

  • You must reapply every year to keep your City of Falls Church massage therapist permit valid. This renewal can be done via the U.S. Mail or in person. You do not need to meet with a city employee to complete this process. Completely fill out the Falls Church massage therapist permit application. Applications must be notarized prior to submission. You may request an application for renewal via email.
  • Provide a copy of your current City of Falls Church business license if you are a self-contractor.
  • Pay the $50 annual application fee to the city treasurer. If paying by check, this payment can be mailed along with the application and a copy of your current certificate to practice as a massage therapist from the Commonwealth of Virginia Board of Nursing to:
